Infinity Rehab’s campus program, Aging in Motion-Successfully (AIMS), allows us to implement a full health promotion model that accommodates the various acuity needs of residents and focuses on improving overall health and preserving independence. Facility operators especially value our health promotion model because it avoids the uncertainties and headaches of trying to achieve this themselves. Today, residents want to retain their strength and independence throughout their life, and our AIMS program directly supports this.

Outpatient Services

The development of outpatient services to serve the independent and assisted living thumbs-upresidents is central to the campus approach. Typically, we are able to begin outpatient services within three months of initiating the program on-site. A turn-key plan, including equipment, space, registration processes, and systems are the cornerstone of the outpatient program.

AIMS Approach

Infinity Rehab’s AIMS approach consists of Physical, Occupational and Speech Therapy Services with specialized fitness programs – creating a Personal Pathway to Health and Wellness.

Starting with individual screening designed to ensure the patient’s highest level of functioning – and addressing Balance, Activities of Daily Living, Physical Fitness and Brain Fitness.

The AIMS program addresses three main areas:

The AIMS program provides early identification and intervention, resulting in reduced falls and reduced resident move- outs for a higher level of care.

Campus Systems

From reviewing emergency call logs to flagging level of care changes, Infinity Rehab establishes systems to capture information regarding residents who may have difficulty in their current living situation. For campuses with either a health clinic or a wellness coordinator, Infinity Rehab staff members develop a cross-referral relationship with the on-site practitioners to integrate a continuum of services.

Several ongoing classes work to ensure rehab involvement with the residents-at-risk:

  • Weekly Parkinson’s class
  • Fall and Balance clinic
  • Walker “tune-up”

Promotion and Education

We also work closely with the campus to promote the AIMS approach to residents through a variety of mediums including brochures, newsletter articles, and utilizing closed circuit television.

Monthly health seminars are held to educate campus residents, staff, families, and possibly prospective residents. We cover a variety of topics – all intended to educate the audience on post-acute rehabilitation and its role in recovery.
