Plane Crash or Car Accident?

This video caught my attention. Though it addresses medical errors in the hospital setting, it made we wonder about medical errors in all health care settings.   Safety is a prerequisite to quality and is defined as freedom from accidental injury (Institute of Medicine, 2000).
